Аннотация: Background: Changes in immune parameters that have a significant effect on the direction of pathological process in patients with chronic viral hepatitis C (HCV) remain poorly understood [1]. Aim: to determine the factors affecting the state of T‐cell immunity and the frequency of liver fibrosis in patients with HCV with a differenПоказать полностьюt genotype. Methods: 156 HCV patients with the 1st genotype and 112 with the 3rd genotype were examined. HCV was diagnosed based on EASL recommendations [2]. The stage of liver fibrosis in patients with HCV was evaluated by ultrasonic elastography on an Aixplorer (France) using the METAVIR scale. In 78 patients with HCV with the 1st genotype, 56 with the 3rd genotype of HCV, and in 46 healthy donors, a study of the subpopulation composition of T‐lymphocytes in the blood was performed by direct immunofluorescence using monoclonal antibodies on a Navios flow cytofluorimeter (Beckman Coulter, USA). Results: The frequency of liver fibrosis F3‐F4 in patients with HCV with the 3rd genotype prevailed over the same indicator in patients with the 1st genotype of HCV virus (41.2% versus 25.9%; OR 0.49; CI 0.28 ‐ 0.86; p = 0.02). Compared with healthy individuals, patients with the 1st and 3rd genotypes of HCV showed an increase in the content of T‐helpers of effector memory (p < 0.001), “terminal‐differentiated CD45RA‐positive effector cells” (p < 0.001) and a decrease in the number of T‐helpers of central memory (p < 0,001). Differences in patients with HCV with the 1st and 3rd genotypes were determined in the subpopulation of T‐killers. In patients with the 3rd genotype of HCV, an increase in the number of naive cytotoxic T‐lymphocytes (p = 0.035), central memory T‐cells (p = 0.04) and TEMRA T‐killers (p = 0.03) was observed compared with healthy people. In patients with the 1st genotype of HCV, such patterns were not observed. Conclusion: The frequency of liver fibrosis F3‐F4 by METAVIR in patients with HCV with the 3rd genotype is higher than in patients with HCV with the 1st genotype. T‐cell immunity in patients with HCV at the 1st and 3rd genotype has differences in the composition of T‐killer subpopulations.
